الأهمية التشخيصية والدور المناعي لمستويات IL-4 وIL-21 في مصل مرضى داء غريفز

يُعدّ داء غريفز اضطرابًا مناعيًا ذاتيًا يتسبب بفرط نشاط الغدة الدرقية نتيجة فقدان التحمل المناعي وتحفيز إنتاج الأجسام المضادة الذاتية. ويُنظر إلى اختلال توازن السيتوكينات بصورة متزايدة بوصفه مكوّنًا قابلًا للقياس ضمن هذه العملية المرضية. الهدف: هدفت هذه الدراسة إلى تقييم مستويات الإنترلوكين-4 (IL-4) والإنترلوكين-21 (IL-21) في مصل مرضى داء غريفز، وتقدير أدائهما التشخيصي باستخدام تحليل منحنى خصائص التشغيل للمستقبِل (ROC)، ودراسة ارتباطهما بمعايير وظائف الغدة الدرقية. الطرائق: استُخدم تصميم دراسة حالة–شاهد، شمل 50 مريضًا بداء غريفز و50 فردًا سليمًا كمجموعة ضابطة. قِيست مستويات IL-4 وIL-21 في المصل باستخدام تقنية الامتزاز المناعي المرتبط بالإنزيم (ELISA) وفقًا لإجراءات الشركة المصنّعة. حُللت البيانات باستخدام برنامج SPSS الإصدار 26، وأُجريت المقارنات بين المجموعات باستخدام اختبار t للعينات المستقلة، وقُيّمت الدقة التشخيصية بتحليل ROC، واستُخدم ارتباط سبيرمان لدراسة علاقة السيتوكينات بكل من TSH وFT3 وFT4 لدى المرضى. النتائج: ارتفع IL-4 معنويًا لدى المرضى مقارنةً بمجموعة السيطرة (16.70 ± 2.17 مقابل 3.40 ± 0.69 بيكوغرام/مل؛ P < 0.001)، كما ارتفع IL-21 (47.81 ± 3.15 مقابل 20.20 ± 3.53 بيكوغرام/مل؛ P < 0.001). بلغ الحد الفاصل لـ IL-4 مقدار 9.5 بيكوغرام/مل بحساسية 100% ونوعية 100% ومساحة تحت المنحنى 1.000، بينما بلغ الحد الفاصل لـ IL-21 مقدار 40.1 بيكوغرام/مل بحساسية 98% ونوعية 100% ومساحة تحت المنحنى 1.000. لم يُظهر IL-4 ارتباطًا معنويًا مع TSH أو FT3 أو FT4. وأظهر IL-21 ارتباطًا إيجابيًا معنويًا مع TSH؛ إذ بلغ معامل سبيرمان 0.368 وكانت P = 0.008، من دون ارتباط معنوي مع FT3 أو FT4. الاستنتاج: يرتفع كل من IL-4 وIL-21 بوضوح لدى مرضى داء غريفز ويُظهران دقة تشخيصية مرتفعة. ولا تدعم نتائج الارتباط علاقة مباشرة بين ارتفاعهما وشدة فرط نشاط الغدة الدرقية الكيميائية الحيوية، لعدم وجود ارتباط مع FT3 أو FT4.
